Person-Centered Care Planning

Person-Centered Care Planning (PCCP) is a holistic approach to planning and providing behavioral health services. Person-Centered approaches guide and organize services to increase consumer engagement in service interventions based on the connections between provider interventions and the consumer’s goals and visions.

The Evidence-Based Practice Center (EBPC) incorporates a Person-Centered Care Plan into all its training in evidence-based and empirically supported models offered by its cadre of consultants and trainers. To enhance providers’ access to and utilization of PCCP in service delivery, the EBPC is pleased to offer multiple training options. EBPC and BHA recommend follow-up consultation and ongoing support for the full implementation of this practice.

  • Practice what you've learned in PCCP introductory trainings. Register for the PCCP Case Studies to join us for the entire series or just those topics in which you would like a refresher or further study. By selecting this package you will be registered in all trainings within the series. Practice what you’ve learned in PCCP introductory trainings. For each topic we will use a sample person-centered assessment to develop an Interpretive Summary and a Treatment Plan focusing on one category or barriers or needs. These highly interactive webinars activities will blend group work, independent work, processing and strategizing working with frequently encountered situations and symptoms.

  • A series of 90-minute webinars designed to enhance treatment planning. Register for the PCCP Key Concepts Series to join us for the entire series or just those topics in which you would like a refresher or further study. By selecting this package, you will be registered for all trainings within the series. PCCP treatment plans become roadmaps, guiding service interventions and serving as useful tools for exploring when and why course corrections may be advised. Join us for a series of 90-minute webinars designed to enhance treatment planning, making treatment plans a useful tool to keep work on track in day-to-day meetings and interventions. Each week we will do a deep-dive into a component of the treatment plan with a special focus on how to engage consumers in the exploration and development.

    Using Crisis Plans as an effective part of counseling to build resilience to symptom increases and life stressors. In addition to being a useful (and in some cases required) document, Crisis Plans can guide important conversations about medication use and changes, self-monitoring and using supports to monitor changes in symptoms and coping, and proactive planning for when additional supports are needed. Products of Person-Centered Crisis Planning include fostering consumers’ skills for self-advocacy and creating records we can use to advocate on consumers’ behalf for times when they are unable to self-advocate effectively.     A case for replacing compliance with Engagement (Training Time: 9:30 am - 11:30 am) As behavioral health services and supports have evolved, one of our most powerful lessons learned is that pressuring “compliance” or “treatment adherence” does not work. We cannot force people into health and wellness. Taking pills and/or attending appointments is not sufficient for achieving wellness. Words like compliance, adherence, and attendance persist in interactions and in documentation. At the same time, problems with disengagement and dropout persist. Research on person-centered care supports shifts to language and approaches that are more empowering and engaging. In this training we will identify common pitfalls and strategies for being more engaging and empowering including effective listening, collaborative decision making, and matching approaches to stages of change.

    Principles of Person-Centered Care Planning (PCCP) applied to using the Managed Intervention Plan (MIP). When participants in Maryland’s Public Behavioral Health System Residential Rehabilitation Program services are at risk of losing their placement, policy calls for the development of a Managed Intervention Plan. The intent of developing this intervention plan is to provide a coaching experience for consumers so that they can be successful in their time with our programs, instead of having to be discharged for rule violations/code of conduct. This training will cover how to engage with consumers to explore with what the alternatives are, how they can best help, and what strategies need to be implemented in order to retain the individual in services. The result of this engaging process should be a comprehensive and actionable written plan to prevent discharge from services. (Training Time: 9:30 am - 12:30 pm)

The EBPC regularly schedules technical assistance sessions for Maryland providers seeking help with completing DLA-20 assessments and putting Person-Centered Care into practice. You are welcome to register for any or all topics of interest to you.
